Abstract

The health care industry in the U.S. is peculiar. We spend close to 18% of our GDP on health care, yet other countries get better results—and we don't know why. To date, we still lack widely accepted answers to simple questions, such as "Would requiring everyone to buy health insurance make us better off?" Drawing on behavioral economics as an alternative to the standard tools of health economics, author Douglas E. Hough seeks to more clearly diagnose the ills of health care today. A behavioral perspective makes sense of key contradictions—from the seemingly irrational choices that we sometimes make as patients, to the incongruous behavior of physicians, to the morass of the long-lived debate surrounding reform. With the new health care law in effect, it is more important than ever that consumers, health care industry leaders, and the policymakers who are governing change reckon with the power and sources of our behavior when it comes to health.

Abstract

In Unmanageable Care, anthropologist Jessica M. Mulligan goes to work at anHMO and records what it’s really like to manage care. Set at a health insurancecompany dubbed Acme, this book chronicles how the privatization of the healthcare system in Puerto Rico transformed the experience of accessing andproviding care on the island. Through interviews and participant observation,the book explores the everyday contexts in which market reforms were enacted.It follows privatization into the compliance department of a managed careorganization, through the visits of federal auditors to a health plan, and intothe homes of health plan members who recount their experiences navigating thenew managed care system.Inthe 1990s and early 2000s, policymakers in Puerto Rico sold off most of theisland’s public health facilities and enrolled the poor, elderly and disabledinto for-profit managed care plans. These reforms were supposed to promoteefficiency, cost-effectiveness, and high quality care. Despite the optimisticpromises of market-based reforms, the system became more expensive, not moreefficient; patients rarely behaved as the expected health-maximizing informationprocessing consumers; and care became more chaotic and difficult to access.Citizens continued to look to the state to provide health services for thepoor, disabled, and elderly. This book argues that pro-market reforms failed todeliver on many of their promises.Thehealth care system in Puerto Rico was dramatically transformed, just notaccording to plan.

Abstract

The health systems we enjoy today, and expected medical advances in the future, will be difficult to finance from public resources without major reforms. Public health spending in OECD countries has grown rapidly over most of the last half century. These spending increases have contributed to important progress in population health: for example, life expectancy at birth has increased, rising on average by ten years since 1970. The challenge now is to sustain and enhance these achievements in a context of tight fiscal constraints in many countries combined with upward pressure on health spending from factors such as new technological advances and demographic changes. Finding policies that can make health spending more sustainable without compromising important achievements in access and quality requires effective co-operation between health and finance ministries. Sound governance and co-ordination mechanisms are therefore essential to ensure effective policy choices. Prepared by both public finance and health experts, this report provides a unique detailed overview of institutional frameworks for financing health care in OECD countries. One of the main features of this book is a comprehensive mapping of budgeting practices and governance structure in health across OECD countries.

Abstract

Is there any justification for the common practice of allocating expensive medical resources to rescue a few from rare diseases, when those resources could be used to treat devastating diseases that affect the many? Does the use of Prozac and other anti-depressants make us inauthentic beings? Is it immoral and irrational to have children? What is the force of examples and counterexamples in bioethics? What are the relevance of moral intuition and the role of empirical evidence in bioethical argument? What notion of “function” underlies accounts of the distinction between normality and disease and between therapy and enhancement? Is there an inherent conflict between research aimed at therapy and research aimed at gaining knowledge, such that the very notion of “therapeutic research” is an oxymoron? The twenty-one chapters in this volume strive, through the use of high quality argument and analysis, to get a good deal clearer concerning a range of issues in bioethics, and a range of issues about bioethics. The essays are provocative, indeed, some quite radical and disturbing, as they call into question many common methodological and substantive assumptions in bioethics.
